When scheduling an appointment with one of our physicians, it’s important that you have the necessary information available. Please click here for a Checklist of Required Patient Information. Once you have your appointment, our goal is to ensure every patient visit to our office is a pleasant and efficient experience. Prior to your visit, please bring the following information with you for your appointment:

Authorization/Referral: If you are enrolled in a managed care plan, your insurance company may require you to obtain a referral or authorization from your primary care physician before your office visit or surgery. If we do not receive your referral in advance of your visit, you will be financially responsible for those services.

Photo Identification: For security purposes, we require you to bring a valid driver’s license or a current/valid photo ID to every appointment.

Insurance Card/Billing Information: Be sure to bring your insurance card(s) to each appointment. As a courtesy to our patients, we will bill your insurance company for office visits, injections, x-rays and surgical procedures. Based on your insurance or plan benefit coverage, it may be necessary to collect deductibles, co-pays, co-insurances and/or a deposit at the time of each visit. Please contact your insurance carrier prior to your visit to obtain complete benefit coverage information. We accept cash, personal checks, debit cards, MasterCard, Visa and American Express. We accept most major insurances, including both Highmark and UPMC Health Plan.

Bring all radiology films or CD (X-ray, MRI, CT, bone scan, etc) for the related injury or condition.

Bring any relevant medical records for prior treatment for this injury.

For knee or hip appointments please wear or bring shorts (with no metal).

For shoulder appointments please wear or bring a tank top or sleeveless shirt.

Our patients with an upcoming appointment may print the Health History Form and Notice of Privacy/HIPAA Form and bring the completed forms to your scheduled office visit. Or you can scan/email the completed forms to registration@tristateortho.com in advance of your appointment. You may also access your e-Chart via our secure patient portal, which will allow you to provide additional medical history information up to seven days prior to your office visit. After your visit, patients can also access office visit summaries.
