Welcome to Tri-State Physical Therapy (TSPT). We want to make every therapy visit an efficient, productive and pleasant experience. All new patients or returning patients with new injuries, please visit the Forms section of this website to complete the new patient paperwork prior to your first visit. Also, please bring the following information with you for your appointment:

Referral/Prescription: Please bring your therapy prescription with you to the first visit. This allows your therapist to customize a therapy program based on your specific condition/injury/diagnosis and your physician’s recommendations, along with your input, to gain the most from your physical therapy visits for an optimal outcome.

Photo Identification: For security purposes, we require you to bring a valid driver’s license or a current photo ID to every appointment.

Insurance Card/Billing Information: Be sure to bring your insurance card to each appointment. We do collect insurance specific co-payments at the time of check-in. As a courtesy to our patients, we will verify and bill your insurance company for therapy office visits or any “durable medical equipment” (i.e. braces, splints, crutches, electrical stimulation, etc.) provided in our office. Based on your insurance or plan benefit coverage, it may be necessary to collect a deposit or co-pay at the time of each visit. Please contact your carrier prior to your visit to obtain complete benefit coverage information.

We accept cash, personal checks, debit cards, MasterCard, and Visa.

Appropriate Attire for your Visit: Please wear clothes suitable and comfortable to perform exercises specific to your injury. This may include shorts, sweats, t-shirts, etc.

For example, if you have a knee problem it is best to wear shorts so we can better evaluate and treat your knee during your visit. A tank top would be appropriate for shoulder injuries. Please always wear comfortable and appropriate shoes.
